If you are currently in the hospital

If you arrive at the hospital or any other facility incapable of treating your injuries and need to be transported to the nearest facility capable of providing the appropriate care, you do not need approval from the Assistance Company.

If you then wish to be transferred to another hospital, your home, need someone by your side or need to have your child under the age of 18 returned home (with an attendant if needed), you must call the Assistance Company at 1-877-984-7437 to receive approval

If you have received a bill and need to file a claim, download and fill out the attached claims form, and email or mail your claim to:
